Disability Services Documentation Criteria

It is the policy and practice of Chippewa Valley Technical College to comply with Section 504 of the Vocational Rehabilitation Act and also the Americans with Disabilities Act. Students must provide written documentation that clearly shows that a disability which interferes with major life activities currently exists.

Disability documentation must:

  • Be prepared by an appropriately licensed Doctor, Psychiatrist, or Psychologist who is familiar with the history and functional limitation of the disability. Reports must be on letterhead, typed, dated, and signed to be considered valid.  Testing or assessment results must be submitted as well.
  • Provide evidence of current impairment as well as a historical record when possible. Information may be obtained from the student’s academic record, assessments, informal assessments, and input from professionals that know the student. A special education director or school psychologist may provide a current psychological evaluation or an older evaluation with a letter stating that there is no change in diagnosis or accommodations. When submitting school records, the most current IEP and Re-eval should be included in addition to any other evaluations or records.
  • Clearly link functional limitations to the individual’s specific accommodation requests. The report should explain and document via formal and informal assessment how the requested accommodations mitigate the impact of their disability on a specific task or activity (i.e. how extended time on an exam accommodates the impact of attention deficit disorder, how a learning disability interferes with note-taking skills and suggesting that a note-taker is needed). In addition, to the extent possible, the report should consider the impact of the disability and also the need for the requested accommodation in a college setting. 
  • Show evidence from the assessment(s) that support each accommodation request. Accommodations should always be individualized and, therefore, a standardized list of requested accommodations should not be submitted.
  • Information meeting the same criteria that documents each disability or impairment must be presented when multiple disabilities or impairments present.


    Disability documentation must generally be less than three years old to be considered current and valid. Any questions regarding documentation should be directed to:
