Disability Services
Reasonable Accommodations and services for academic and accessibility needs are available for students with documented disabilities in accordance with the Americans with Disabilities Act standards. CVTC buildings are accessible to people with physical disabilities. Auxiliary aids, support services, and assistive technology are available based upon individual needs.
Disability Documentation Requirements are not a part of the admissions process to the college and do not affect application to a program.
Requests for Accommodations should be made to the Disability Services Office as soon as possible so that arrangements for accommodations may be initiated. Students will be required to submit medical or other diagnostic disability documentation that supports the specific request prior to receiving accommodations (see documentation forms below). Providing this documentation of disability is the responsibility of the student. Documentation should be recent, relevant, and comprehensive. If the original documentation is incomplete or inadequate to determine the extent of the disability or reasonable accommodations, the college has the option to require additional documentation.
Any information submitted regarding a disability is treated as confidential information as defined by the Family and Education Rights and Privacy Act (FERPA). Questions regarding the appropriateness of the request and/or documentation required should be directed to the Disability Services Office, (715) 833-6234.
For information about making the transition from high school to postsecondary education, click on http://www.dpi.wi.gov/sped/pdf/tranopndrs.pdf "Opening Doors to Postsecondary Education and Training". While in high school, juniors and seniors receiving special education services may find it useful to review the "Opening Doors" document. Students should print, complete, and retain the appropriate documentation form below so the information is readily available for future disability documentation purposes.
Provided below are documentation forms that can be printed for your convenience. The documentation requirements are based on recommendations from the Education Testing Service Policy Statement for Documentation of Disabilities in Adolescents and Adults http://www.ets.org/ and are similar to those used by UWEC.
