Prepare Your Packets

If you are applying to more than one department, each department needs its own application and corresponding certifications in a packet. Place the documents in separate envelopes with the department name and your name on the outside.

Department Packet should contain:

• Application
• Resume
• Copy of driver's license
• National Registry EMT Basic/Paramedic Certification and/or copy of State EMT License OR evidence of enrollment in course with estimated certification date
• Wisconsin Firefighter I or II Certification
• Hazardous Materials Certificate (if Haz-Mat Technician)
• Human Resources Information
• Supplemental Application (Marshfield & Red Wing Only)

CVTC Packet should contain:

• Human Resources Information

• Verification of completion of CPAT after 10/29/2011

• If you do not have a valid CPAT include the following:

• CPAT Registration form.

• CPAT Practice Waiver, if taking the CPAT but NOT interested in practice sessions

• One $100.00 check to CVTC  OR credit card information on CPAT Registration Form
