How do I take my boards?

First, you must apply for Graduation--you can apply online. You need to indicate in the comments section whether the intent is to graduate after one year or the intent is to "earn" the LPN but continue in the two-year program.

After you have applied for Graduation, you can fill out the online registration on the State of Wisconsin Department of Regulation & Licensing http://drl.wi.gov/prof/burhealth.htm , or pick up a packet for HEC 100F (Linda Londre).

How do I fill out the application?

Directions for completing the Licensure Packet to apply for the State Board Examination.

  • Watch the licensure video on the CVTC e-stream system
  • Type in www.cvtc.edu/estream
  • Select Archived Webcasts (towards the top of the screen)
  • Select Nursing Licensure (from the list)
  • Select Player Preference (212 K preferred)

The video is about 30 minutes long and will walk you through the packets.

The statement of graduation form MUST be filled out and given to the program director. The statement of graduation form is sent in to the State Board of Nursing by the Program Director after you have completed all program requirements and/or graduated.

What is the Learning Resource Center? How do I contact them? 

The Learning Resource Center (LRC) exists to support classroom and clinical instruction within the healthcare programs. The Resource Center provides a place for students to practice and develop their skills in a ‘safe environment’ prior to patient care in clinical.

Hours: Monday through Thursday (8:00 AM to 6:00 PM)

What happens if I fail a class?

Nursing Program Clinical Re-entry Evaluation Policy

Progression/Withdrawal/Dismissal/Program Reentry

Throughout the program, students are graded according to their ability to meet objectives. These objectives are based on graduate competencies outlined by the National League for Nursing. Students are required to meet the objectives in theory, skills, and clinical courses. To progress in the program, students must meet the established grading standards.

Students are encouraged to take all courses in the Nursing Program according to the sequencing structure found on the Program Requirement Sheets. Most of the courses in the program have prerequisite courses that must be taken in the sequence identified.

If a student interrupts his/her program prior to completing first semester nursing courses, the student may be required to reapply to the program. If the student interrupts his/her program after first semester, a new program planning sheet must be developed and the student will be admitted on a space-available basis. This will delay program completion. Repeating a nursing course that requires clinical placement or affects clinical sequence will be contingent upon clinical openings as determined by the Nursing Program Director. Students must meet with a program counselor and apply for the R-list through the Admissions Office for re-entry into the program. Students are offered clinical placement based on their place on the R-list as openings occur. Because timely re-entry into clinical courses cannot be guaranteed, students should seek advice from the program counselor or course faculty when considering withdrawing from a course or otherwise making changes in their course schedules. Students who stop attending class and do not complete course assignments, yet do not officially withdraw, will receive a failing grade in that course.

A student may appeal dismissal from the Associate Degree Nursing Program. To begin the appeal process, a student must submit a written appeal to the Nursing Program Director. This letter should explain why the student feels s/he should be given an exception to the progression policy, include a description of any extenuating circumstances, and identify the changes the student is making to ensure his/her success. The Program Director will schedule an appeal hearing and notify the student of the time and location. The student, program counselor, and nursing faculty are invited to attend the review. The Nursing Appeals Committee considers three criteria when reviewing the appeal.

  • Extenuating circumstances that may have contributed to the student’s difficulties.
  • Evidence of a realistic plan of changes to increase the student’s chance for success (because students who continue doing what they were doing will continue to get the same results).
  • Likelihood of success if the student were given another chance.

Students will be granted only one opportunity to appeal for re-entry into a nursing program. Students who are not successful in any program course after meeting with the Nursing Appeals Committee will be dismissed from the program with no further opportunity to appeal for re-entry into a nursing program.

What do I need to do if I want to sit for LPN Boards and continue on in the Associate Program?

Those ADN students who wish to apply for the PN technical diploma may do so with two options:

  • Graduate at the end of the first year and receive the Practical Nursing diploma. The student is removed from the program and changed to undeclared.
  • Earn the technical diploma without receiving it. The diploma is just noted on the transcript so the student can work as an LPN while seeking the Associates Degree. The student is not awarded the degree nor removed from the program until she/he actually graduates.

In both cases, the student needs to complete and submit the graduation application form so we know an audit is needed. Everything is going fine up to that point. What's not happening is the student indicating in the comment section of the form whether or not the intent is to "stop out" completely and graduate or just "get recognition" with the technical diploma.

It is imperative we know WHY the student in the ADN program has applied for the LPN technical diploma. They need to indicate in the comments section whether the intent is to graduate after one year or the intent is to "earn" the LPN but continue in the two-year program.
