Instructions - Caregiver Law Requirement
As of October 1, 1998, background checks are mandatory for all persons who seek to be employed in the care giving industry. This new law includes students as contracted care providers. The State of Wisconsin has identified specific crimes and offenses that will bar or limit individuals from employment in the health care or child care field or prohibit them from obtaining a license or certification.
The current list of crimes that may bar you from being a health care/child care provider are available for your review in the Information Center and Counseling Center at the Eau Claire Clairemont Campus, Menomonie, Chippewa Falls, River Falls, and Neillsville Campuses.
The following steps must be followed before your application can be considered for program eligibility:
- Complete the "Search for a Record on:" section only of the Wisconsin Criminal History Single Name Record Request.
- Read the Background Information Disclosure Instructions.
- Complete, date, and sign the Background Information Disclosure Form.
- Complete, date, and sign the CVTC Release Form in the presence of a witness who will also date and sign the form.
- Prepare a check payable to "CVTC" in the amount of $18.00 (non-refundable fee).
- Get a complete set of instructions and forms.*
- Return the completed forms and $18.00 check to:

The results of your criminal history record request will be sent to the Admissions Office at Chippewa Valley Technical College. The original will be provided to the program director of the program for which you are applying. If your criminal history record request is returned with "records attached", you will be asked to schedule an appointment with a CVTC program counselor to determine the impact that the crimes listed will have on your career plans.
Please note:
If you have resided outside of Wisconsin in the last three years, you MUST also request a criminal background check from the state or states in which you previously resided. A list of out-of-state agencies to contact for criminal background checks is available by calling the Admissions Office at 715-833-6246 or 1-800-547-2882, ext. 6246.
