Learn From the Pros:
For many people, motorcycling is a fun and energy-efficient means of transportation or recreation. However, motorcycling requires skill, concentration and reasonable precautions. Although it's possible to learn to ride a cycle on your own, trial and error is a tough teacher of motorcycling skills. That's why virtually all motorcycling groups have endorsed the Motorcycle Safety Foundation's Basic Rider Course (BRC) for beginning riders and the Experienced RiderCourse for advanced riding skills.

If you are under eighteen (18) years old and want to apply for a motorcycle permit, it will be necessary for you to do the following PRIOR to going to the DMV to take your written exam:
     1. Register and pay for the class.
     2. Call 715-855-7521 to request a copy of Form MV3575. You must bring this form
         with you to the DMV.

MV3575 is the ONLY proof of enrollment form the DOT will accept.
THIS FORM IS REQUIRED ONLY FOR PERSONS UNDER THE AGE OF EIGHTEEN (18) WHO WISH TO OBTAIN THEIR PERMIT PRIOR TO ATTENDING CLASS or
