Application for Admissions:We prefer you complete the online application. Here is a paper version if you need it.

CPR Information:All CVTC Health Occupations programs require CPR. Read this information to be sure you take the correct CPR training.

Criminal Background Check forms:Wisconsin law requires students that will be doing internships or clinical placements in health care, child care or related occupations to have a criminal background check completed.

Permission to Exchange Information form: Students applying to the Interwoven Accounting and Business Management program with Lakeland College need to complete this form.

Program Change form:This simple form is used to change your major or Program Choice. If you have not yet attended the college, it is very important that you use this form to change your program choice rather then re-submitting a new application. If you have attended before, but not in the last year, you should submit a new Application for Admissions.

Residency Determination form: This form must be completed by students from other states except Minnesota. Minnesota residents are treated the same as Wisconsin residents for purposes tuition and fees.
