Steps for Admission

  • Phase I – Complete all steps in Phase I to be accepted as a pre-program student. Students who have pre-program status have been accepted into the college but not into the program. Pre-program status may be granted because admissions requirements need to be satisfied, the program is filled to capacity, or the program start date is in the future. A pre-program student may be eligible for financial aid and can register for general education courses, but they are not able to register for core Nursing-Associate Degree courses (543 numbered).
  • Phase II – If not completed at the time of application, complete all steps in Phase II to become program eligible or for placement on the waiting list.
  • Phase III – Students who needed to complete Phase II requirements will need to submit the Core Course Eligibility Form when the last Phase II requirement is satisfied to alert Admissions of program readiness.

Pre-Program Requirement (Phase I)

  1. Submit an application for admissions located at cvtc.edu/apply
  2. Pay the one-time $30 application processing fee
  3. Submit ONE of the following transcripts:
    • Official high school transcript which includes all courses taken, grades received, and date of graduation
      • If you are a current high school senior, please submit a provisional high school transcript to satisfy this requirement. CVTC will request you to submit a final high school transcript at the end of your senior year.
    • Copy of General Education Development (GED) certificate
    • Copy of High School Equivalency Diploma (HSED)
    • Official transcript showing completion of an Associate Degree, Bachelor’s Degree, or higher degree.
  4. Meet ONE of the assessment ranges - submit ONE of the following by providing proof with a transcript or score sheet. You will be considered eligible for pre-program status even if one of the below options is too low.
    • Minimum high school/GED/HSED GPA of 2.8
    • Minimum college GPA of 2.2 (6 or more credits)
    • Associate Degree or Bachelor's Degree
    • ACT® composite score 23
    • ACCUPLACER® (Next-Generation) Assessment composite score of 263. The composite score consists of Reading, Writing and Quantitative Reasoning, Algebra, and Statistics (QAS). ACCUPLACER® is an un-timed, computerized test. To schedule an ACCUPLACER® appointment, visit our website: cvtc.edu/assessment or contact Student Central at 715-833-6200. You will be subject to a testing fee.
Compass® and ACCUPLACER® (Classic) scores are accepted as well. Contact Student Central at 715-833-6200 for additional information.
  5. Complete a Wisconsin Criminal Background Check and Background Information Disclosure Form. For instructions to request a background check, please go to cvtc.edu/backgroundcheck. A hardcopy instruction packet can be supplied upon request by calling 715-833-6200. A $36 fee will be required at the time of your order. Background checks processed for other entities will not be accepted.
Please note: Your processed criminal history results are considered current for admission purposes for four years. If you are still enrolled as a student or re-enter CVTC after the four-year expiration date, you will be required to request another background check and pay the required processing fee. Also, once enrolled in core courses, you may be asked to complete another background check to meet certain clinical site compliance agreements.

Core Program Requirement (Phase II)

  1. Meet and submit proof of one of the assessment range scores in Phase I. In order to be eligible for the core program, you must provide a transcript or score sheet that shows you have met the minimum required.

  2. Submit proof of completion of 543-300 Nursing Assistant training that includes a clinical component.  Acceptable documentation is a Nurse Aide Directory card, a transcript or a diploma from a technical college, training agency or nursing home. You do not need to be a Certified Nursing Assistant but must submit proof of successful course completion.
NOTE: The Nursing Assistant class does not qualify for financial aid.
  3. Complete 806-177 General Anatomy & Physiology (4 credits) with a grade of "C" or better ("C-" is not acceptable). If this course was taken at a college other than CVTC, submit an official transcript showing final grade and credits earned to CVTC Admissions.

  4. Submit proof of current and active enrollment in 806-179 Advanced Anatomy & Physiology (4 credits) (registering for the course does not count as currently and actively enrolled as the class needs to have started for it to meet the requirement) or completion of the course with a grade of "C" or better ("C-" is not acceptable).
    • If you are currently enrolled and actively taking this course at CVTC, we will be able to see your current enrollment if this is not your last item. However, if you meet all all other requirements as outlined before you begin your current and active enrollment in the Advanced A&P course, then you will need to submit a Core Course Eligibility form as outlined in number 11 below to inform us you have now satisfied this requirement.
    • If you are currently enrolled and actively taking this course at a college other than CVTC, you will need to submit proof of current and active enrollment with an unofficial transcript to CVTC Admissions (admissionsmail@cvtc.edu). If this course was taken at a college other than CVTC, submit an official transcript showing final grade and credits earned to CVTC Admissions. If this is your last Admission Requirement, you will need to complete the Core Course Eligibility form as outlined in Phase III, number 11 below.
    • Note: The Advanced A&P course is still a course that is required to be able to progress and graduate from the core Nursing-Associate Degree program. If you satisfy the Advanced A&P Admission requirement with current and active enrollment, but it is later determined that you did not achieve a solid C grade or higher, you will have to work with your academic advisor to determine next steps to satisfy the course in a timely manner.
  5. Complete the HESI A2® Exam – HESI A2® scores of 75 or higher in each area of the following areas:  Anatomy & Physiology, Math, and English are required. The English score is a composite of grammar, reading, and vocabulary. You will be subject to a testing fee and the number of times you can test is limited. It is recommended that you take the HESI assessment after you complete General Anatomy & Physiology. For more detailed information about CVTC's HESI requirement and to schedule a testing appointment, go to cvtc.edu/HESI.

Core Course Eligibility (Phase III)

Applicants who meet all requirements in Phase I and II will be accepted or placed on a waiting list as of the date all documentation is received.

11.  Core Course Eligibility Form – Applicants meeting one of the scores or options listed above in Phase I and completing Phase II requirements must submit a completed Core Course Eligibility Form (CCE Form) to the Admissions Office.

    • For those satisfying the Advanced A&P requirement with current and active enrollment and this is your last Admissions Requirement, submit a Core Course Eligibility (CCE) Application Form as soon as possible by going to the:
      • CCE Application Form page found in your Commons account and clicking on the "Forms" drop-down menu, clicking on Forms, and then "Core Course Eligibility Form" under Admission.
      • Click on the "Core Course Eligibility Form" button.
      • Next, choose the middle radio button of "Other admission requirement (e.g. Exam, transcript, licensure, etc.)" and choose the "Next" button.
      • Complete the CCE Application Form for your desired semester start date and choose the "Submit" button at the bottom.
    • For all other academic course requirements where current and active enrollment is not an option, you will receive an email prior to the end of each term through student email with the CCE Form submission date for that term along with direction on how to submit it.
    • On the form applicants should indicate the semester and year they will be prepared to begin Nursing-Associate Degree core courses. Applicants are placed on waiting lists by date of eligibility regardless of the semester and year requested.
    • Students completing all of Phase I and Phase II requirements will be accepted or placed on a waiting list only when the Core Course Eligibility Form is on file.
